Once selected for a Kirkwood Study Abroad program, students are required to complete a Health Self-Assessment Form. The purpose of this form is to help Kirkwood provide appropriate assistance to you should the need arise during your study abroad experience. International Programs must be advised of all health issues to make any necessary preparations. This information will be kept confidential and will only be shared with appropriate professionals as it relates to your health and safety. In some cases, students may be asked to complete an additional assessment form to be signed off on by a medical provider that can best advise students on recommendations for their participation. Kirkwood Study Abroad Programs does not require you to have a physical exam before going abroad; however, we do recommend that you schedule a visit with your medical provider to discuss any concerns.
